Bodywork has a 24 hour cancellation
policy. Since our services are time intensive,
you are essentially paying for a particular time
slot. For Cancellations,
please make sure you have read the
Cancellations
Policy. If you still wish to cancel, please email
or phone 24 hours in advance. For

For credit and
debit card customers this charge may be assessed
automatically.

Late arrivals will NOT receive an extension of
services. Please arrive at least 15 minutes
prior to your first appointment to complete
new client paper work and the initial consultation.

Our professional staff is
committed to providing excellent services. Gratuity
is not included in the service prices, but is always
appreciated.

Children under the age of
15 will not be permitted
to have services. Children 16 to 18 must be
accompanied by their legal guardian.

Rates are subject to change.

All gift certificates are
non transferable,
replaceable, nor refundable.
All gift certificates are valid for 6 months unless
stated on the Gift Certificate otherwise.

All services are guaranteed. Your therapist
will check in with you throughout your session to
ensure your satisfaction. If you are not pleased
with your services, please use this time to make your
therapist aware, and your service will be of no
charge.

Full payment must be
received when services are rendered. Payment
may be made via personal check, cash, credit or
debit card.

Returned Check, denied Credit or Disputed Charges will incur
additional charges: Returned check and denied
Credit is $25.00 per occurrence. Disputed charges will be investigated. |

OF PRIVACY PRACTICES FOR PROTECTED HEALTH INFORMATION.
Law requires the privacy of your health information be maintained and
that you are provided with notice of the legal duties and privacy
practices with respect to your health information. Other than the
uses and disclosures, your health information will not
be sold or provided to any outside marketing organization. We must
abide by the terms of this notice and we reserve the right to change the
terms of this privacy notice. If a change is made, it will apply
for all of your health information in our files, and you will be
notified in writing.
